    ORGANIZATION: Citizen Care, Inc.
    DEPARTMENT: Walden Senior Center
    POSITION: Program Specialist
    STATUS: Full Time; 40 hours / week
    LOCATION: Robinson Township
    HOURS: 8:00 AM to 4:30 PM; Monday through Friday

    Citizen Care, Inc., a subsidiary of Partners For Quality, Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit, accredited organization and have locations throughout the West Hills and South Hills of Allegheny County.
     
    The Program Specialist, internally referred to as a Services and Supports Coordinator, is responsible to the Director for Learning / Employment / Retirement Services for the successful implementation of individualized plans for services and supports for people who attend the Walden Senior Center and Training and Outsourcing Center. Duties include, but are not limited to:
    • The coordination of services and supports
    • Focus on CQL’s Quality Measures
    • Compliance with CCI’s Policies and Procedures
    • Compliance with 2380 and 2390 Regulations as determined by service facility
    • Compliance with the Department of Aging Regulations as determined by service facility
    • Performance of all ISP responsibilities of the Program Specialist position in accordance with the 2390 Regulations
    • Compliance with the Department of Human Services’ medication administration guidelines
    • Quality Improvement / Assurance Planning
    • Completion of all responsibilities in a manner consistent with the Agency’s Vision, Mission and Values
    • Personal / Professional development
    • Other duties as specified
     
    QUALIFICATIONS
    1. Master’s degree or above from an accredited college or university in Special Education, Psychology, Public Health, Rehabilitation, Social Work, Speech Pathology, Audiology, Occupational Therapy, Therapeutic Recreation or other human services field; OR
    Bachelor’s degree or above from an accredited college or university in Special Education, Psychology, Public Health, Rehabilitation, Social Work, Speech Pathology, Audiology, Occupational Therapy, Therapeutic Recreation or other human services field AND one (1) year experience working directly with disabled persons; OR

    Associate’s degree or completion of a 2-year program from an accredited college or university in Special Education, Psychology, Public Health, Rehabilitation, Social Work, Speech Pathology, Audiology, Occupational Therapy, Therapeutic Recreation or other human services field AND three (3) years experience working directly with disabled persons; OR

    License or certification by the State Board of Nurse Examiners, the State Board of Physical Therapists Examiners, or the Committee on Rehabilitation Counselor Certification or be a licensed psychologist or registered occupational therapist AND one (1) years experience working directly with disabled persons
    1. One (1) year management/supervisory experience preferred
    2. Experience in designing and implementing services and supports plans
    3. Excellent communication and writing skills
    4. Excellent references, both work and personal
