    ORGANIZATION: Citizen Care, Inc.
    DEPARTMENT: Learning/Employment/Retirement Services
    POSITION: Program Specialist
    STATUS: Full Time; 40 hours / week
    LOCATION: Coraopolis
    HOURS: 8:00 AM to 4:30 PM; Monday through Friday

    Citizen Care, Inc., a subsidiary of Partners For Quality, Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit, accredited organization and have locations throughout the West Hills and South Hills of Allegheny County.
     
    The Program Specialist, internally referred to as the Services and Supports Coordinator, is responsible to the Director for Learning / Employment / Retirement Services for the successful implementation of individualized plans for services and supports for people who attend the Senior Citizen Center, the Center for Creative Opportunities, and / or Center For Community Supports including:
     
    • The coordination of services and supports
    • Focus on CQL’s Quality Measures
    • Compliance with CCI’s Policies and Procedures
    • Compliance with 2380 Regulations as determined by service facility
    • Compliance with the Older Adult Daily Living Center Regulations 6 PA Code / Chapter 11 as determined by service facility
    • Performance of all ISP responsibilities of the Program Specialist position in accordance with the 2380 Regulations
    • Compliance with the Department of Human Services’ medication administration guidelines
    • Quality Improvement / Assurance Planning
    • Completion of all responsibilities in a manner consistent with the Agency’s Vision, Mission and Values
    • Personal / Professional development
    • Other duties as specified
     
    QUALIFICATIONS
    1. Master’s degree or above from an accredited college or university and one (1) year of work experience working directly with persons with disabilities; OR
    Bachelor’s degree from an accredited college or university and two (2) years of work experience working directly with persons with disabilities; OR
    Associate’s degree or 60 credit hours from an accredited college or university and four (4) years of work experience working directly with persons with disabilities;
    1. One (1) year management/supervisory experience preferred
    2. Experience in designing and implementing services and supports plans
    3. Excellent communication and writing skills
    4. Excellent references, both work and personal
